Chronic disease management plan is a familiar term for structured GP care for an ongoing health condition. Australia's Medicare arrangements changed in July 2025, introducing GP chronic condition management plans. These plans help coordinate goals, treatment and appropriate referrals, but eligibility and funded services depend on the current rules and individual circumstances.
Why does the terminology matter?
An older GP management plan or team care arrangement may still appear in your records. It is important to check how current transition rules apply rather than assuming every old document must immediately be replaced. Your usual practice can review existing plans and explain the appropriate next step.
A plan is more than a form for obtaining subsidised appointments. It should identify your health priorities, agreed actions and the professionals involved. Bring a list of current conditions, treatments and what you want to improve in daily life.
For the Australian guidance behind this explanation, see Services Australia: Services available under a GP chronic condition management plan.
Does a plan make allied health free?
Not necessarily. Eligible services may attract Medicare benefits, but a provider can charge more than the benefit. Referral requirements, annual limits and service eligibility apply. Ask the allied health provider about fees and check which services the plan supports before attending.
The number and type of funded visits should be confirmed using current Services Australia guidance. A plan does not automatically authorise any test, specialist visit or therapy you choose. Clinical relevance and administrative eligibility are separate requirements.
Can it be arranged through any online service?
Do not assume so. These plans are often best coordinated with a regular GP who knows your history and can arrange ongoing reviews. Ask the provider about availability, eligibility and continuity before booking specifically for a plan.
Doccio's online doctor service can be checked for its current scope, but this glossary entry does not promise plan preparation or Medicare funding. A doctor decides what care is appropriate and whether another practice or in-person appointment is needed.

Where to go next
Read online doctor consultations to understand the relevant service. A doctor decides whether a consultation, examination, referral or another pathway is appropriate for your situation.
