Gap fee and out-of-pocket cost
Gap fee and out-of-pocket cost explained in plain English: what it means, what to ask and how it relates to Australian online care.
By Doccio Clinical Team · Sources reviewed September 2026
In everyday healthcare billing, a gap fee often means the difference between a provider's charge and a Medicare benefit. Your out-of-pocket cost is what you ultimately pay yourself. Medicare programs can use more specific definitions, so ask which meaning applies when a provider discusses a gap. Start with the actual fee and eligibility
Ask the provider for the total charge, any expected Medicare benefit and the remaining cost. If no benefit applies, the full private fee may be your out-of-pocket expense. Do not subtract a rebate you have seen advertised for a different service.
Confirm whether you pay the entire fee first or only a balance. Keep receipts and claim records so you can reconcile the amounts. A pending benefit is different from a confirmed payment into your account.
Do not combine unrelated charges
A doctor's consultation and a pharmacy supply are separate transactions. Tests, imaging and specialist appointments may also have independent fees. A low gap for one part of care does not establish the total cost of everything recommended afterwards.
If a clinic uses “no gap”, ask exactly which service that describes. It should not be interpreted as a promise that every later provider will charge nothing. Clear questions at booking can prevent surprises without changing the clinical decision.
How to read Doccio pricing
Doccio publishes private pay-per-consult fees. Use the current pricing page as the starting point and do not presume a Medicare rebate. If the service confirms a different billing arrangement in future, check the details for your selected appointment. Doccio’s published service pricing
If affordability affects whether you can proceed with a care plan, tell the clinician. They can consider appropriate alternatives or pathways, while administrative staff explain billing. Ask for the explanation in writing if the amounts remain unclear. A glossary definition can help frame the question, but it cannot calculate your final cost without the actual fee, eligibility and service information.
