An injury report records the circumstances and consequences of an injury, often for an employer, insurer or healthcare provider. It is different from a medical certificate and may be completed through a workplace process. Reporting requirements and deadlines vary by jurisdiction and scheme, so accurate facts and prompt notification matter.
What information should I record?
Write down when and where the injury occurred, the task being performed, what happened and any witnesses. Describe symptoms and immediate action without guessing a diagnosis or assigning blame. Keep relevant photographs or correspondence where appropriate and lawful.
Tell the employer through the required process as soon as practicable. A clinical appointment does not automatically notify the workplace or lodge a compensation claim. Ask which form is required and who needs a copy.
For the Australian guidance behind this explanation, see safeworkaustralia.gov.au: Table 3.4: Prescribed time periods for injury notification.
What is the clinician's role?
A clinician assesses the injury, documents findings and may provide medical evidence about function or capacity. They may distinguish what you reported from what they directly observed. That distinction helps create an accurate record rather than implying the doctor witnessed the event.
Bring the relevant form and explain your actual duties. Some injuries require examination, imaging or treatment that cannot be provided online. A report should not delay care for an injury that is clinically urgent.
Is an injury report the same as a claim?
No. Internal notification, regulatory reporting and workers compensation claims are separate processes with different responsibilities. An employer may have specific duties for serious incidents, while a worker may need to meet a scheme's claim requirements. Check the relevant state or territory authority rather than applying another jurisdiction's deadline.
Doccio's online doctor service can be checked for its current scope, but this page does not promise workplace injury reporting or scheme-specific certification. For major injury, severe bleeding, head injury with deterioration or loss of limb function, seek urgent care rather than a routine online appointment.
Where to go next
Read medical certificates to understand the relevant service. A doctor decides whether a consultation, examination, referral or another pathway is appropriate for your situation.
