Short answer

A GP referral to a specialist generally lasts 12 months from the first specialist attendance under that referral, unless another duration is specified. A specialist-to-specialist referral generally lasts three months. Different arrangements and exceptions exist, so check the document and receiving practice rather than assuming every referral expires identically. Start with who wrote the referral

A referral from a GP and one from a specialist do not necessarily have the same standard period. Services Australia sets out the relevant Medicare referral rules, including the usual 12-month GP referral and three-month specialist referral periods. The written document may specify a different applicable period. Services Australia: Referrals for specialist treatment

Find the actual letter before trying to calculate expiry from memory. Note who issued it, whether a duration is stated and which condition or course of treatment it addresses. A booking reminder alone may not include enough information to answer the question.

The first attendance matters

For the standard GP referral period, the relevant starting point is generally the first specialist attendance under that referral, not simply the day the GP wrote it. This distinction matters when there has been a long wait between referral and the first appointment. Services Australia: Referrals for specialist treatment

Ask the specialist's reception team which referral is recorded for your course of care and when it commenced. If you have seen more than one clinician, do not assume all visits used the same document in the same way. Accurate dates prevent unnecessary renewal requests and missed requirements.

Can a referral last longer or indefinitely?

Healthdirect explains that a referring doctor may specify a longer period or an indefinite referral when clinically appropriate. This is a clinical decision, not an option a patient can add to the document themselves. The referral should reflect the expected care needs. Healthdirect Australia: What is a referral?

An indefinite referral does not mean every future health problem is automatically covered. A new or unrelated issue may need a new referral or further assessment. Ask the receiving practice and referring doctor how the document applies to the appointment you are planning.

What if the specialist asks for a new referral?

Ask why it is needed. The existing period may have ended, the appointment may concern a new problem or the practice may need updated clinical information. Knowing the reason helps the referring clinician address the correct request rather than produce a vague replacement.

Book assessment early enough to allow time for records or examination if necessary. A new referral is not simply a date-changing service. Explain any change in symptoms, treatment or circumstances since the original letter so the doctor can decide what should be included. Can a referral be dated after the appointment?

Do not assume a later document can retrospectively fix every Medicare requirement. Referral and billing rules need to be met for the actual service. Contact the specialist's practice before the appointment if the referral is missing or uncertain. Services Australia: Referrals for specialist treatment

Tell the referring clinician the true timeline. They should not be asked to make a document appear to have existed earlier than it did. If you have already attended, ask the practice or Services Australia about your specific billing position rather than relying on a general online answer.

What should I check before the next visit?

Confirm that the receiving practice has the referral, that it applies to the planned consultation and that the patient's details are correct. Ask about fees separately. A valid referral does not establish that the specialist bulk bills or that every service will receive a Medicare benefit. Services Australia: Referrals for specialist treatment

Keep the letter and first-attendance information somewhere you can find them. If you change specialists, read using a referral with another specialist and check the new practice's intake requirements.

An online doctor consultation may be a way to discuss referral needs when clinically appropriate. The doctor decides whether an updated letter is justified and whether more information is needed. Do not wait until the appointment day if the practice has already asked you to clarify the referral.
